46. Elastic Rhinoplasty

A long nose can be shortened and the tip reshaped by implanting an elastic thread through a 2 mm incision.
Elastic lifting of the nose has not only an esthetic objective, but also a functional purpose. Indeed, by correcting the nasolabial angle, this procedure improves the patient's breathing. The indication for this simple procedure is established during the preoperative examination. The nose-tip is pinched between two fingers and is drawn backwards and upwards; if it is raised easily, the elastic thread can be implanted.
The result is optimal and stable over time.

47. Elastic Lifting of the Upper Lip with three threads

A long upper lip is an esthetic defect of familial origin. If the lip is not excessively long, it can be lifted by means of two or three elastic threads in order to reduce its length.
The preoperative design is drawn from the wing of the nose to the contralateral margin of the columella. From here, it descends obliquely to the extremity of cupid's bow, follows the edge of the vermilion border and then travels back up to the wing of the nose. This design is reproduced on the contralateral portion of the lip. The pathway of a third elastic thread, extending halfway down the lip, is then drawn. Regional anesthesia is carried out.

48. Mastopexy of drooping breasts with silicone implants, by means of two tiny incisions and elastic threads

Today, the appearance of breasts that have silicone implants can be improved through a single 8 mm incision in the areolar margin. Simply by encircling the areola with an elastic thread, which is mounted on a two-tipped Jano needle, we can conize the breast, reposition the implant in contact with the chest wall, reduce the diameter of the areola or make it symmetrical, and lift the breast by a few centimeters. By means of a second, 2 mm long, incision in the axillary column, mastopexy of breasts with silicone implants can also be carried out.
This ambulatory Elastic Plastic Surgery procedure can be used both to give normal breasts a more natural look and to correct the so-called “ball-in-a-sock" appearance of larger breasts.
